Assume you work for Dr. Samuel Zinzer (0000-856425-00). Using Manual Claims Cards provided by your instructor and today’s date as the service date, create claims for the following. (Note: Some codes may need to be looked up in the Schedule of Benefits):
- Ian Leighton (HC: 9987 542 658 DF DOB: 12-20-1982) comes in for his annual physical. The doctor performs a urinalysis and takes blood while he is there (3 marks).
- Patti Jones brings her daughter Peyton (HC: 8754 654 521 QW DOB: 05-04-last year) for a periodic checkup. (1 mark)
- Yvonne Houston (HC: 5487 326 256 PT DOB: 07-14-1990) wants to have an IUD inserted. The doctor does a partial gynaecological exam with pap smear while she is there. (Hint: An IUD is a form of Family Planning) (5 marks)
- Dr. Zinzer visits Roger Williams (HC: 9578 236 485 TY DOB: 08-22-1930) at his home at 1500hrs (3pm) to assess his congestive heart failure. The doctor sacrificed office hours for the visit. (3 marks)
- Ken Neilson (HC: 8891 215 652 HT DOB: 01-01-1975) comes in with foot pain. The doctor performs a brief exam and quickly diagnoses bunion. (1 mark)
- Emma Pignioli (HC: 7854 652 654 QW DOB: 10-04-1962) comes in for her annual flu shot. The nurse gives her the immunization. (2 marks)
- Larry Girard (HC 4814 542 156 RE DOB: 05-28-1950) is due for his allergy shot. The nurse performs the service. (1 mark)
- Sarah Moukled (HC: 7884 251 845 KL DOB: 07-31-1948) is seen for respiratory problems and fever. The doctor performs a thorough exam of her respiratory tract and takes vitals. He diagnoses her with upper respiratory tract infection. (1 mark)
- Winifred Brown (HC: 8452 456 547 PO DOB 03-18-1962) is a diabetic patient, who hasn’t been feeling well. The doctor performs an assessment of her diabetes, and has the nurse complete a dipstick urinalysis and take blood while she is there. (3 marks)
- Kyle Conner (HC: 8542 215 362 GH DOB 06-26-2007) comes in for his annual physical. (1 mark)
- Lacey Smithers (HC: 8756 325 215 MV DOB: 08-04-1968) comes in for a one-hour counselling session regarding her anxiety. (1 mark)
